I'm not entirely sure he always left with three, considering the fact he ALWAYS forgot to grab his pistol. He always leaves with his jacket, tool/investigation kit, and most likely his hat. Although he didn't always bring his hat.

Well, he is most known for his magnifying glass (although it is not referred to very often by Conan Doyle) as well as his Calabash pipe, deer stalker cap, and trench coat (the latter was never mentioned in the books). However, he also owned many books (e.g. encyclopedias, dictionaries, fact books) that he often referred to for information on various subjects for his cases. Oh, and you can't forget the violin he always played while thinking over a mystery.
