Some Capitol children were trapped behind the gates of President Snow's mansion when a bomb went off, and they were all injured. A team of medics came to help them, Prim included. They didn't think another bomb was going to go off. But it did. The last thing Katniss saw of her sister was Prim with a terrified look in her eyes as she burst into flames.
Katniss had a mental breakdown. Earlier in the book, Gale had designed a two-shot bomb: A bomb that would explode once, then when everyone thought they were safe, would explode again. Katniss knew it was Gale's invention that inadvertently killed her sister, and though she still always loved him, it was never the same again.
Primrose dies when she comes to treat Capitol children after the rebels bomb them using silver parachutes. Some did not blow up and when she arrived, they exploded and killed her.Then the Capitol hovercrafts send down more bomb parachutes, Primrose then completely bursts into flames.
prim was killed by plutrach and his people...they set bombs and prim was helping snows soldiers heal so she and the other doctors died(her mom got out safely).
katniss still thinks that snow killed her though but shes sort of 50-50 on both sides.
