There is no book 4 of Avatar. It ends at Book 3: Fire. He doesn't die at all.
He's not the last Avatar, he's the last airbender. He is the last airbender. The Avatar's soul is reincarnated into the next bender in the cycle, so if he were to die, the Avatar would be reborn into the next cycle. As the cycle is air, water, earth, fire, he would be reborn into one of the water tribes. However, as Avatar Roku tells Aang in the Avatar State, the first episode of the second season, "If the Avatar is killed while he is in the Avatar State the cycle will be broken, and the Avatar will cease to exist." As Aang is killed in the Avatar State in "The Crossroads of Destiny," the Avatar Cycle has likely been broken. Indeed, Aang reveals in the third season that he can no longer travel to the Avatar State, which indicates that he is not only the last airbender, but possibly the last Avatar as well. But then he does manage to go into the Avatar State in the finale and everything is happy. Yay.
When Aang goes "berserk" he goes into something called the "Avatar State" what it is, is all the power, emotions, and experience of all the past Avatar's (Aang's past lives) being summoned into Aang's body, he can only enter this state when he is experiencing immencily strong emotion, or pain. When he is in the this state he is at his most strongest.. but he is also at his most vunerable, the reincarnation cycle does not apply when he is in this state, and if he were to die while he was in it, the Avatar cycle would be broken, a new Avatar would not be born after his death, and the Avatar would cease to exist.

When the Legend of Korra starts, you see that Korra proved to be the Avatar. Of course, in order for there to be a new avatar, the old avatar must die. So the second Korra was born, Aang died.
