Sherlock Holmes smoked shag tobacco, kept in the toe of a Persian slipper in his pipe. He kept his cigars in the coal-scuttle. He also smoked cigarettes on occasion. He called a very difficult challenge a 'three pipe problem.'

More correctly Sherlock Holmes smoked "Black Shag", Shag is the cut of the tobacco. Others Holmes may have smoked as they are in a recreation of Holmes study in England- Arcadian Mixture, Birdseye (Watson's favorite) Odd one was Honeydew ( Irish Style) offered to Holmes by Susan Cushing.

He often smoked cigarettes when out and about, and once in a while he smoked a cigar. The pipe was reserved for smoking at home.

He smoked shag tobacco, a rough cut. He kept it in the toe of a Persian Slipper on the mantel as we are told in 'The Musgrave Ritual,' 'The Naval Treaty,' and 'The Adventure of the Empty House'. It was in the sitting room of his flat in 221B Baker Street.

No, he did not. He smoked a shag tobacco kept in a Persian slipper on the mantel. He evidently did visit a opium den during his detective work, though whether he actually smoked any opium was not stated.
I don't believe so, although he did disguise himself as an opium taker in "The Man with the Twisted Lip".

He did, however, inject himself with cocaine on various occasions, took morphine, and smoked.
Sherlock Holmes was never an opium addict. Holmes was known to use a 7% solution of cocaine. He did this when there was no case to occupy his brain.
Upon reading one of the later books in the series, "The man with the twisted lip", I recall reading a conversation between Holmes and Watson, with Holmes introducing the matter in the form of '...I suppose Watson that you imagine that I have added opium-smoking to cocaine-injections and all the other little weaknesses on which you have favoured me with your medical views...' displaying perfectly the cause for question without a confirmed answer. I believe the thought of Sherlock Holmes and opium to be merely put together in regards to seek the clues of a case and nothing of recreational purpose.

Sherlock Holmes was depicted by Arthur Conan Doyle as being a pipe smoker, hence addicted to nicotine, and also an occasional user of cocaine. At that particular period in history, cocaine was not illegal.

Holmes drank tea, and this is mentioned in a number of the stories. Little detail was provided with that tidbit of information about the great detective, however.

Perhaps at the same tobacconist where Holmes gets his shag tobacco and Watson gets his cigarettes, the nearby Bradley's in Oxford Street. (The Hound of the Baskervilles)
