Inactivity is one sign. Also, If it has diseases, such as tail-rot, Icthyophthirius, (both which are very difficult to cure) and other aquatic parasites, it is very likely to die. And also, fish cannot live infinitely. Most bettas will live to be 3-4 years, however mine died at age 8 because of old-age.
