Jem had broken his arm at the elbow during his struggle with Bob Ewell. Dr. Reynolds was trying to examine him, and Jem kicked him. Dr. Reynolds then gave Jem a medicine that knocked him out, so that the doctor could examine the arm properly and provide some treatment.
Dr. Reynolds is the town doctor in Maycomb in the novel "To Kill a Mockingbird" by Harper Lee. He is called to the Finch household when Jem breaks his arm, and he sets the injury and treats Jem's pain. Dr. Reynolds is known for his skill as a doctor and his no-nonsense approach to treating his patients.

Dill persuades Jem to do lots of things, maybe that he should not be doing, such as touching the Radley house. He does give Jem male companionship during the summer though, which is good. But, he does influence Jem poorly towards tormenting the Radleys.
Jem is upset with Mrs. Dubose's racist comments towards his father, so he retaliates by destroying her flowers as an act of rebellion. Jem is trying to stand up for his family and assert his own sense of justice in response to Mrs. Dubose's bigotry.
