Lennie was touching Curley's wife's hair when he went too far, and touched her too hard. She screamed, and he grabbed her, telling her to be quiet. He did this because he really wasn't supposed to see her. George told him not to see her because she was "bad news". He grabbed her so people wouldn't know that he had been with her. George said being with her was a bad thing, and if Lennie did another bad thing, he wouldn't be allowed to tend the rabbits. He kept on holding her, and accidentally cracked her neck.

Lennie accidentally killed Curley's wife by breaking her neck in a struggle after she allowed him to stroke her hair in the barn. He didn't mean to hurt her, but his lack of understanding his own strength and the situation led to the tragic accident.
