I dont know the exact way it happened but in tracing my lineage I have found that in England at least as population grew more and more people were getting the same first name, especially in the father to son instance or being named after a royal relative. These people had where they were from added to their name casually at first to differentiate from other people with the same name such as William " of " Cornwall, which later became the surname Cornwall. In other instances they were identified by their trade such as William " the " Smith or William Smith ( person who works metal ). You will probably find that one of Margaret Thatchers' early relatives Wove baskets , Weaver was another such name. With many names you can see right away where they came from. The middle name was added in order to further distinguish between people of the same first and last name.
