I would say that 90% of the time when someone says this, they are speaking with a spouse or significant other about their discontent with the relationship. In this context, "they've moved on" means that they have discovered that they weren't happy and have started trying other things with other people. It doesn't typically mean that they are involved with someone else already, or even dating anyone else. It just means that they are tired of being in that relationship and want something else. Obviously, because of all the complexities of the English language, there are less common uses also. 'Moving on' can obviously mean that a person is on a journey, has rested, and literally 'moves on' toward the destination. However, aside from a few minor uses, the first explanation would be the primary meaning.
