I believe they are medical doctor that has special training to work with other complementary alternative medicine pracitioner.
While I've never done meditation on stimulants, I have some perspective on this as I happen to be a psychiatrist who also practices Zen. I think for someone who has legitimate medical problems with attention a stimulant such as Adderall can be very helpful for meditation. However, if used improperly or used without the careful direction by a competent physician, it can be harmful to both a person's health and to meditation. My personal feeling is also that self-directed use of a stimulant for purposes other than a legitimate medical need would also violate the precept on abusing intoxicants.
There are many cultures that have unusual medical practices. Some of these cultures include the French, the English, as well as the Italians.

Medical practices from the 1500s and 1600s included bloodletting where leaches were placed on a person to withdrawal blood. There weren't many dental practices except for extractions.
