replace one side at a time, take the bolt out that goes through the mount and jack motor up on that side and replace the mount.

Use a jack with a board on top to support the engine. Loosen the bolts for the motor mount. Raise the engine slightly then take the bolts out and remove the motor mount. Install the new mount, tighten the bolts, then remove the jack.
Loosen the motor mount retaining bolt. Jack the motor up a few inches. Remove the motor mount retaining bolt and the motor mount bushing. Reverse the process to install the new motor mount.
With the car front end safely lifted, support the engine with a jack, using a board on top. Unbolt the motor mounts one at a time. Slightly lift the engine, then remove and replace the motor mount. Repeat for the other side.
By "disassemble" I assume you mean replace. Jack up front of car and secure. Remove (1) mount to chassis nut on bad mount, use jack to raise engine enough to remove (2) mount to block bolts then remove the old and replace with new mount. I think they cost between $17 and $25 at the parts store unless you want the poly type.

I know from expirience that the manual trans has two. One is called the right trans mount, near the motor, and the other is the rear trans mount located directly under the trans. You can use a pry bar the lift up on the trans and see if the mount is broken. the rear mount is about fifty bucks, and is easy to replace, its mounted with only four or five bolts.
