Continuous means the action is ongoing. It never stops.

Continually means the action is repetitive, but it does stop before it starts again.

For example, if it's raining all night, the rain would be falling continuously on the roof.

But, say little Johnny has a habit of forgetting to cross his T's in his homework, you could say that he continually forgets to cross his T's because the problem is recurring. Johnny, however, is not constantly forgetting to cross his T's every minute of every day.
