There is no such thing as a 'neutral phase'. 'Live' or 'hot' conductors are called 'lines', whereas the neutralconductor is at approximately earth (ground) potential.
So, a toaster would be connected between a line and a neutral conductor.

Usually yes. A typical 480 volt panel is a 3 phase panel with 480 volts line-to-line and 277 volts line-to-neutral. However, I did once see a panel that was 480 volts, 3 phase, but because it served only motors it did not have a neutral. (a 3 phase motor doesn't use a neutral.) Similarly, if a panel uses only 2 legs of a 3 phase 480 volt system, which would be called single phase, it would not require a neutral if it only feeds 480 volt single phase loads. But I find the idea of no neutral to be extremely unusual and in my one personal experience, I blamed it on the age of the system. In 16 years of commercial and industrial construction I have never installed a 3 phase panel without a neutral and all my work is designed by engineers.
In a single-phase circuit like a house fed from 4-wire three phase in the street, the current flows in the live and neutral wires. The neutral currents from a street of houses are expected to cancel each other so that the overall current in the neutral leaving the transformer is small.
One pin for phase other pin for neutral and third one is for earth/ground wire. In India the right pin is for phase, left pin is for neutral and pin on top side is for ground wire. This third top side pin is slightly bigger and longer compared to phase and neutral pins.
It would happen in case of 3phase domestic use where consumer side load balancing is not being done and there is loose connection in the neutral wire. Due to the unbalance and poor neutral, the system neutral will be shifted and un balanced voltages will be recorded in all the 3 phases of the house. The phase loaded with much utilities will indicate less voltage and other will have high voltage. Gadgets connected to this phase will be damaged.
In a Wye/star system the overloads look at the phase to neutral/ground current; not phase to phase current. Which is always calculated as root three or 1.73205.............etc. Therefore the overloads have to be set to the phase to neutral/ground current.
