Unstable people have always existed. In the past they were either locked up or kept at home and as such the incidence of their violence was significantly reduced.
However, that doesn't account for all of the psychotic behavior that we see. In years past, some of the unstable people would not have been locked up, they would have been considered normal enough that society would have allowed them out, but without the news reporting the violence, that particular group of unstable people would not have been "inspired" to seek the attention that is given by a bloodthirsty media. The media even has a name for the phenomenon, it's called "copycat murders", but they refuse to accept any responsibility for their actions. Unstable people are willing to die, so long as they can get a few minutes of fame in the process.
And last, the past DID have it's share of psychotics running around doing terrible things. But recognize that the rest of the population had firearms and knives and could take care of problems if someone started to threaten innocent lives. In a world where so many people are frightened of firearms, we are all forced to run around unarmed and unable to protect ourselves. That makes it easy for the mentally unstable people to do as much damage as possible.
Also, drugs and alcohol can influence violence.
