"How many anuses do you have?" is "¿Cuántos anos tiene?"
However, the question "How old are you?" is "¿Cuántos años tiene?" The difference is the tilde, the squiggly line over the n. It has a "ny" sound, like canyon. When you answer about your age, you say "Yo tengo (some number) años" only if you say it without the tilde, you're indicating a major medical problem and your Spanish friends will snicker.

You would say "El cumpleaños de mi hermana" (with a tilde, the little wave, over the 'n' in 'cumpleanos'. Without the tilde it would sound like "counting anuses" (in Spanish)
