the whole door panel has to be removed, (pop the plastic behind the handle and remove the screw, pop the lock lever and remove the screw, and pop the window control handles off and pull up on the door panel) then on the back of the door panel there is a bunch of tabs bent down to hold the grill on, unbend them and the speaker grill will ease its way out.

You have to remove the bracket assembly (that holds the grille) from the header panel.
remove the screws usually behind the door handle, also remove the cover tabs on the door grab on the panel and then remove the speaker grill and remove the three or four screws around the speaker not the ones holding the speaker in place.

On most vehicles, there will be nuts or bolts inside the door the mirror is located on, but under the door panel. If there is a triangular panel separate from the door panel, they may be under this. Otherwise, you will have to remove the door panel to access them.
I have a 2002 Trailblazer it took me forever to figure this out. To remove the headlamp assembly on a Trailblazer, remove the plastic panel that covers the hood latch assembly, remove the grill then release the retainers for the headlamp assemly. The bottom retainers for the grill are tricky and difficult to release. you can break the grill if done incorrectly. I'm told there is a way to change the bulbs without removing the grill but it seems to involve bending the grill in half and some severely scraped knuckles.
you remove it by taking the whole panel off - it is a moulded piece which is integrated into the door panel. there are small plastic screws - 2 located on the side of the panel and two located on the opposite side of the panel - just undo and carefully take the panel off. There is also a screw by the buttons for the electric windows. Hope this helps
remove the door panel, there are 2 bolts that you can get to through holes in the inside steel panel. remove 2 linkages. the handle will then come off from the outside.
