When it opens up, you will be in the Fill Effects menu, that's where you can add pattern and texture to your background.

If you mean how to add design to the background go to "Design" on PowerPoint 2007 and choose your theme.
Fill Effects
First, you need to choose a background image. Then, choose the slide, or slides, you would like to add a texture to and click the "Design" tab. Choose the texture you like and, if you want it for one particular slide, just click "Close". If you would like to add the texture to the entire presentation, click "Apply to All", and your presentation is ready to go.

It depends on what you are trying to achieve:If you are trying to add an image to use as a reference (it will not show in the output renders), go to View (in the menu just above the workspace)-> Image Plane-> Import ImageIf you are trying to add an image as a background, for instance, and want it to show in the output, add a polygon or nurbs plane (depending on your preference, I suggest use the former) and apply a texture to it with the target image and scale it correctly using the UV Texture Editor (Window-> UV texture editor)
