Each PUK code is unique to it's SIM card. Not the phone itself or the service provider. If your a registered member on their site, you can log in and get your PUK code from there. It should be on the top row saying "unlock my SIM card". Make sure your phone is on first. If you can't, call their costumer service. NOTE: Don't incorrectly enter your PUK code for more than 10 times or you'll have to buy a new SIM card.
