There is a Microsoft article about this very subject. If you are unable to locate the buttons described in these instructions, the entire Microsoft Article is available in the 'Related Links' section on the lower portion of this page. First, you must set the options to allow for form creation. Click on the 'Microsoft Office Button' (the circular button at the top left of the window), and click 'Word Options' at the bottom of the resulting menu. From the 'Popular' section (on the left side), place a check in the box labeled 'Show Developer tab in the Ribbon' and click on OK. Next, click in the document where you would like for the checkbox to be. Click on the 'Developer' tab at the top of the screen. Then, click on the 'Legacy Tools' button. This button is located in the 'Controls' group - the fourth icon over on the second row. Mouse over each icon to display what the button does if you are not sure. On the drop-down selection window, there are two checkboxes available - the one in the 'Legacy Controls' section is probably sufficient. Click it, and a checkbox will appear where your cursor was in the document.
Check-boxes are "dingbats," characters that are part of special typefaces. Your version of Word should include either the Wingdings or Zapf Dingbats font (or both).
There are several check-box options you can create simply by changing your font to Wingdings/Zapf Dingbats and typing a lower-case q, r, o or p. Lower case x creates a box with an x in it, to indicate an item is not an option.
If you have Webdings, you can create a plain check-box by typing lower-case c.
