Some times the foramen ovale does not seal. Then you have blood flow from left atrium to right atrium. So there is more blood flow through the lungs. This may lead to pulmonary hypertension, with it's sequels.

When the septum has a hole, it is referred to as a septal perforation. This can lead to symptoms such as nasal congestion, nosebleeds, and whistling sounds while breathing. Treatment options depend on the size and severity of the perforation, ranging from observation to surgical repair.

The septum is the membrane dividing the right and left chambers of the heart.

the right auricle and right ventricle contain de-oxygenated blood and the left auricle and left ventricle contain oxygenated oxygenated blood.

If the septum is ruptured the oxygenated and de-oxygenated blood mix and causes Methemoglobinemia also known as the "Blue Baby Syndrome". this reduces the capacity of the blood to carry oxygen and can cause death. It can also destabilize the blood pressure and cause cardiac failure.

However this is now curable by the Blalock-Taussig Shunt.
