That sort of thing happens if you weren't pierced at an ideal angle (i.e. in this case, probably too much of an angle; piercings work best when they enter and come out perpendicular to the skin). If they come out at too much of angle relative to the surface of the skin, then the skin may try to grow over. It also is possible that your piercer did a perfectly good job, but that's just what your particular body does in reaction to a belly button piercing.

Or, wait a minute... Since you say it's growing over the BALL, and not the other part of jewelry (I was picturing a ring; I forgot for a minute that most women don't wear rings in their belly button piercing), it sounds like the only way that that could possibly happen is if your jewerly is way too small/tight! Get yourself some jewelry of the proper size and that won't happen! If you got it done recently, it might be the case that the jewelry you have is the proper size for later but not big enough to accommodate current swelling.

Probably just a blemish on that ball. Balls like that are usually used with special CBR's that screw on one side and are indented on the other side to meet the flat end of the ring. Not a big deal!

This is a case of misuse of aftercare products or poor aftercare, you need to take your piercing related issue to your body piercer and ask them what is going on. It sounds like a hypertrophic reaction where the tissue is forming a bubble over the piercing. This is caused by the aftercare product being used so much that the body is reacting to it and forming this "hypertrophic reaction", some will stupidly call it a "keloid", which it isn't.

But none the less it's your piercing and your piercer needs to learn how to fix these issues, so go back and see them and see what they say. I have both my lip and belly button pierced. I had my belly button pierced with a hoop and my lip pierced with a bar/stud. The belly button was more irritating then the lip because a hoop piercing requires the piercer to clamp it shut and fiddle around with a new pierced part instead of just screwing a ball on. I recommend any new piercings to be with a bar/stud as it is quicker and less irritating. Put it this way, I'm going for a 2nd lip piercing and not another belly button piercing ;)

I'm not sure what part you're confused about or if you're using some other type of jewelry maybe, but I hope that helps! PS: Also, don't change it earlier than your piercer said you could!
