It depends on how far back you go in history and what cultures you're looking at.

Before modern disposable pads women would use rags, before then often they would just bleed and the clothing they wore would hide the bleeding when they were out of the home. Going back to ancient Egypt people were using papyrus, also moss and natural sponge as tampons - chances are this sort of thing was used a lot. In some cultures women would go to huts where they would bleed freely on the ground, sometimes this was negative seeing menstruating women as dirty or dangerous and sometimes this was positive seeing menstruating women coming together to share knowledge. Some women would bleed without anything, learning to control flow release (menstrual release).

Go back far enough and women wouldn't need to use anything, they'd have spent most of their reproductive years pregnant or breast feeding, and thus they would not menstruate at all.

A LONG LONG TIME AGO, women used torn up rags. Have you ever heard the phrase, "On the rag". That meant a girl was on her period. They'd literally put rags in their underwear.
