Best bet is to see your local professional body piercer and have them look at it for you, they can tell you if it's migrating or rejecting.
You usually can tell if you see the bar through your skin. If you see it take it out before it rips thru.

I got my belly bar pierced about four months ago and at first fitted a 12mm bar (after a month). Gradually, I noticed more of the bar was showing and went down to an 11mm. I then saw a belly bar in River Island which I brought, not knowing what it was made of, but hoping for the best that my belly would not get infected. I recently took it out and went back to my 11mm bar, and have noticed that about 2mm of the bar is showing. Is my belly bar getting rejected by my body? Also, I've never had a problem with it being infected, it's occasionally gone a little red, or sore if I've catched it, and rarely has small amounts of puss, but there is no pain and appears to be no swelling. I clean it regularly with drops of piercing solution and sometimes take it out to clean it properly. Should I be worried that there is a little bit of puss sometimes? It may have just been swelling. If the skin between the exit holes is shorter than 1/4" then its rejecting. At that point, just remove the jewelry and let the piercing heal. It may just be that the piercing migrated a bit. if its red,itchy, flaky or calloused then its probably rejecting. If you're really worried, go see your piercer.
I got mine pierced about 2 months ago, they do'nt actually use a belly bar to pierce it, they clamp it, then shove this stick thing through then pull that out and put the belly bar in.

Your body may be rejecting it. The rejection process is the same as if you had a splinter, your body senses a foreign object and pushes it out.
