Try this simple fix. Put the window down then put it all the way back up. Over time and a few bounces the rear window settles down just a bit and this triggers a safety that prevents the defroster from running with the window down, only it's not down enough for you to notice it.

If you can make the fan run by turning the speed switch to position 5 (hi speed) and make the headlights work by switching them on manually, replace the ignition switch in the steering column to cure this problem. It sounds odd, but that's the fix. Own one, been there, fixed that.

Either a break in power to the rear (i.e. fuse, relay, cut wire etc) or check the contacts on the rear window. They are prone to dislodge where they connect to the window.
