Most likely it's caused by warped brake rotors. Heat and time gets them all eventually.

Why does car shake when applying brakes?

If the steering wheel shakes, but to car doesn't, when applying brakes - your front rotors are warped (if you have disc brakes)...if you have drum brakes, the drum could be cracked or the pads worn unevenly If the whole car shakes, and the steering wheel only seems to react to that, when applying brakes - see above statement, but apply to the rear end. If the whole car shakes, including the steering wheel - all four corners are bad, and you're an accident looking for a place to happen soon. In any case, go to your nearest service center ASAP.

Why does car shake when applying brakes even though front and rear brakes rotors and drums were replaced shakes when pulling emergency brake as well bearings and tires are good?

Good question. You may have an antilock brake circuit that is not sensing the brakes correctly and improperly correcting. Anti lock pulses the brakes when you apply them. I don't know if this is perfected or not.

What could cause the a truck to shake when applying the brakes on a 1998 dodge ram?

Warped rotors or drums and also have the front end checked as well as bearings. Warped rotors being number one, if it's in the steering wheel. If it's more in the seat concentrate on the rear brakes system.

Recently changed front brakes on 2004 Chrysler 300M. Now I have very bad wheel shake when applying brakes and a slight vibration just driving.?

Vibration when applying brakes is most likely warped rotors. Another cause could be loose caliper bolts. The vibration while driving is most likely a tire issue ie (out of balance, broken belt in tire, bent rim). Other possible causes are loose/worn suspension and/or steering parts.

If a car shakes when in dive on a automatic transmission vehicle when accelerating and applying brakes what could be wrong?

Shaking when accelerating and braking show a front end issue. Check the lug nuts to make sure your tires are bolted on tight. Check for play in front end components, like the wheel bearings and tie rod ends. While possibly causing the vibration, the front brakes usually only cause a shake when braking.
