There could be many reasons. Not enough coolant or bad flow, faulty thermostat, low oil. If the coolant freezes then it will overheat. If your car continues to overheat then you should have the coolant flushed and thermostat replaced.
The engine coolant reservoir ( which has a 50 / 50 mix of antifreeze and distilled water from the factory ) is located in the engine compartment on the passenger side

To put in more antifreeze in a 1995 Plymouth Voyager, located the reservoir near the radiator. Antifreeze is no longer added directly to the radiator. It is put into the plastic reservoir that is labeled radiator or coolant overflow.
You can but then the mixture will turn into gel and your entire coolant system will be ruined. If you continue driving your engine will overheat and your car, destroyed.
