Fixing a leak depends on where the leak is and the severity. An exhaust connection could be loose or a gasket is bad. If there is a hole in a pipe there is a possibility that the hole can be welded, taking care to stay away from fuel lines or anything flammable.

A muffler going bad can be caused by many things. But generally, it is either contact with the ground causing a dent, oil burning causing sludge and residue in the muffler, or corrosion from age and moisture in the exhaust pipe causing a hole or the collapse of baffles.
If the exhaust pipe, muffler, or tail pipe have come loose, you will certainly hear the rattling, but that would not be the cause of the shaking. It is something else causing the shaking, ranging from bad tires to bad engine performance.
