No Apollo mission had any casualties. The Apollo 1 Fire was during testing, not a mission. The Apollo 13 explosion was tough... but no one died.
During the Apollo missions, three people died on the actual spacecraft themselves. A total of six people died during these missions if you include workers not involved with the Apollo crafts themselves.
Yes, all Apollo missions returned safely to Earth. No human has ever died in space (though there have been astronauts that died on re-entry).
No, Alan Shepard passed away in 1998 at the age of 74. He was the first American to travel into space as part of the Mercury program but did not participate in the Apollo missions to the Moon.
Three astronauts died in the Apollo 1 space accident: Virgil "Gus" Grissom, Edward H. White, and Roger B. Chaffee.
As of 2021, there have been a total of 18 known fatalities in space-related incidents. These deaths occurred during missions or training exercises involving astronauts or cosmonauts.

No, no Russians have died on the moon. The only humans to have died in space missions were three American astronauts in the Apollo 1 incident on January 27, 1967.

The Apollo 7 spacecraft flew to test the Apollo spacecraft after the first one burnt i a fire. And astronauts Virgil grissim, edward White and roger Chaffee died.

During the Apollo program, the United States sent 9manned missions to the Moon. These nine missions included 24men. Of these 24 men, 12 actually walked on the surface of moon. The other 12 astronauts have been to lunar orbit and back, but did not land on the surface. This included Apollo 8, Apollo 10, and the ill-fated Apollo 13, as well as the 6 Command Module pilots for the six landing missions. The 9 missions to the moon:Apollo 8 - orbitedApollo 10 - orbitedApollo 11 - landed 20 July, 1969Apollo 12 - landed 19 November, 1969Apollo 13 - orbitedApollo 14 - landed 5 February, 1971Apollo 15 - landed 30 July, 1971Apollo 16 - landed 20 April, 1972Apollo 17 - landed 11 December, 1972The men who walked on the Moon :Apollo 11 - Neil Armstrong - Apollo 11 - landed 20 July, 1969 ("One small step for a man, one giant leap for mankind")Buzz Aldrin - Apollo 11 - landed 20 July, 1969Pete Conrad - Apollo 12 - landed 19 November, 1969Alan Bean - Apollo 12 - landed 19 November, 1969Alan Shepard - Apollo 14 - landed 5 February, 1971Edgar Mitchell - Apollo 14 - landed 5 February, 1971David Scott - Apollo 15 - landed 30 July, 1971James Irwin - Apollo 15 - landed 30 July, 1971John W. Frank Borman - Apollo 82. James Lovell - Apollo 83. William Anders - Apollo 84. Thomas Stafford - Apollo 10(John Young and Gene Cernan also orbited with Apollo 10, but each walked on the Moon on a later mission, as above)5. Michael Collins - Apollo 116. Richard Gordon - Apollo 127. Jack Swigert - Apollo 138. Fred Haise - Apollo 13(James Lovell orbited for a second time with Apollo 13, becoming the only man to go to the Moon twice and not walk on the surface)9. Stuart Roosa - Apollo 1410. Alfred Worden - Apollo 1511. Ken Mattingly - Apollo 1612. Ronald Evans - Apollo 17(see the related link)

As of now, no women have died in space. The only deaths that have occurred in space have been male astronauts during space missions.

There were three "Greek god" named series of US manned space missions; the "Mercury" series of 7 single-astronaut capsules, a dozen or so "Gemini" two-astronaut missions designed to to basic theory testing for space docking and maneuvering, and a series of "Apollo" missions which would culminate in landing men on the Moon. The first few were basic testing missions. In the Apollo 1, three astronauts died when a fire began in the pure-oxygen atmosphere. A couple were unmanned tests, and a couple were restricted to Earth orbit. Apollo 8 was the first mission to circle the Moon, and brought our first "live" view of the farside of the Moon. Apollo 11 was the first Moon landing. As a boy growing up in the 1950's, I was always certain that I would live to see the first men on the Moon. Now as an old man in 2012, I certainly hope I won't have lived to see the LAST American to land on the Moon! But there have been no manned lunar missions in almost 40 years, and none are now planned....
The Apollo 1 accident in 1967, where three astronauts died in a cabin fire during a launch pad test, led to significant safety improvements in the Apollo program. The investigation into the accident resulted in redesigns to enhance crew safety, such as changes to materials and procedures. Ultimately, the lessons learned from the Apollo 1 tragedy contributed to the success of the later Apollo missions that landed astronauts on the moon.
