None! Of seven manned Apollo missions to the Moon, using 21 astronauts all together, six missions were successful. Twelve astronauts successfully landed on the Moon, explored it, and returned to earth.

Apollo 13 was our greatest successful failure, in that it did not complete its mission but did return its three astronauts. This is all easily researched at www.NASA.gov. I hope this question is from someone very, very young, because it betrays a complete lack of understanding of history.

No astronauts have died on the moon. The only fatalities in the history of space exploration occurred during the Apollo 1 mission in 1967, when three astronauts died in a cabin fire during a pre-launch test on Earth.
