Varies place to place. Anywhere from 3,000 feet to 22,000 feet. At Chicagoland Skydiving Center where I jump at they have 5,000, 9,000, 14,000, and 18,000 foot jumps. Sometimes once a year they will do a 22,000 foot jump. Just depends on where you go.

Skydivers typically jump from an altitude of around 10,000 to 15,000 feet above ground level. This allows for a free fall of about 60 to 90 seconds before the parachute is deployed. The exact altitude can vary depending on factors such as weather conditions and the type of jump being performed.

That depends on where you live and what type of course you want to do. I skydive at a centre in Lincolnshire, UK called Skydive Hibaldstow.

They charge £230 for a tandem skydive which is where you are harnessed to a tandem skydiving instructor. However this course does not allow you to follow a qualification route, its simply a joyride so to speak.

To begin the route to get qualified, you need to take an Accelerated Freefall course level 1 (total course has 8 levels). This costs approximately £350.

Of course these prices are subject to change and at the time of writing (Aug 2010), they are correct. Either way you should be able to find a skydiving course which is suitable and enjoyable for you.
