If the noise sounds like a banging sound it could be that you have a worn timing chain. Note: 22re's have timing chains not belts. You should have the noise checked-out by an experienced Toyota mechanic.
The correct gap for the 22RE (4 cylinder) is .031
There are many reasons, loose head bolts- improper torque sequence- improper timing of the camshaft-poor valve seal at the seat.

The fuel filter on an 87 4runner with a 22re is bolted to the block under the intake on the passenger side. It's a little difficult to get to, but it can be done without removing the fuel injection system.
I have an 86 4 runner and a 22re engine. The nuts and bolts on mine are 12 mm.
