In the following order:
* Have the wheels SPIN balanced. While they're at it they will be able to determine if a wheel is bent. * Check the front end for excessive wear including the tie rods, ball joints, struts (if applicable), idler arm (if applicable) and rack and pinion (if applicable). * Check the CV joints and half shafts for wear and/or damage (if applicable) One of the above MUST be At Fault with the problem you describe.

Can you elaborate a little? How do you know it's the transmission? Describe the vibration. Is it only at 45 mph? Does it vibrate when you let off the accelerator?

To reduce forearm injuries, hands should be placed on the lower half of the steering wheel, with knuckles on the outside and thumbs streched along the the rim of the steering wheel. Elbows should be able to rest comfortably by your sides.
You have a few options here: Get your wheels balanced-cheapest, have a mechanic check your steering components for wear-possibly more expensive or third, drive faster than 45mph everywhere-probably most expensive(fuel and possible speeding tickets)and the least safe. :)
