UPDATE: The first person's answer is WRONG. Sadly, several have been to the moon. The majority died, either from being suffocated, while in space, or on their way back. Fortunately, at least humans had the dignity to anesthetize them first, but like I said, the poor things never had a chance. Human astronauts, of course, are glad that it was the monkeys, not themselves, who died for the sake of space exploration. That said, there were finally a pair of monkeys who successfully survived the entire trip and were recovered. This was in May of 1959. Their names were Miss Baker and Able.

To the best of my knowledge, no monkey has ever gone "to the moon" in actuality. Several monkeys have been passengers in space travel, but I don't believe any of them have ever gotten beyond low earth orbit.
The most famous "Space Monkeys" in the US are probably Abel and Baker, the first monkeys to successfully return to Earth after traveling in space (59 miles high; at the time the US considered "outer space" to begin at 50 miles rather than the 100 mile limit used today).

Well acording to space the first thing to go to the moon/space was a monkey :D To do a test, the test was that if the monkey would come back down or not!! You would not like to go to space and never come back so they sent a monkey up to test
