Does it have 2 lines coming from the top of the oil filter, and going to the radiator. If so that is the oil cooler. You need to replace the RADIATOR Those 2 lines go into the radiator so it can cool the oil, and the tubes are leaking oil into the coolant.
If there are lines coming from the transmission to the radiator, it has a cooler in the radiator. If there are lines from the transmission to an external heat exchanger, that would be an auxiliary cooler.

There is usually a noticeable aroma of anti-freeze coming from the exhaust, or bubbling in the radiator with the engine running. Also, there is sometimes oil in the radiator or anti-freeze in the oil. A compression test usually tells the story.

If it's just blowing out when you start it you probably have a cracked head or blown head gasket. If there is oil in the radiator there is a change you have oil coming in from the transmission cooler. But there would have to be a lot of oil to make antifreeze come out the radiator cam. Since you did not mention oil I would suspect the head gasket. If you got it hot I would lean toward a cracked head.
if it is a creamy color coming from a radiator look in the oil by looking at the inside bottom of the cap,if it is creamy too,then you have a blown head gasket. that creamy color is water and oil mixed oil mixed in with the water, you got a blown gasket

If it's soft, creamy, probably emulsified oil; suspect a bad head gasket, especially if there are traces of oil in your radiator fluid, or a steamy smell coming from the tailpipe. Sorry!
If the smoke is coming from the electric fan itself, then it needs replacing. It could be the smoke is steam that the fan is drawing from the radiator, and you have a coolant leak at the radiator.
