could be a bad sensor a bad gauge or a broken or disconnected wire

Bad gauge and/or coolant temp sensor The coolant temp sensor doesn't control the gauge. There is a sensor in the head on the driver's side that controls the gauge. It is fairly easy to get to, probably one of the only things on a 4th gen. lol
Your temp gauge is a very simple circuit with few things to go wrong. Your temp sensor may be bad, the gauge maybe bad, or the connecting path may be bad. Sometimes if you have taken the gauge pack out, it does not seat right when it is reinstalled. The connection inside the dash is all pressure and prayer.
that is rather unusual i would ask the dealership you brought it from and ask them and then go from there. Some vehicles have two temp sending units, one for the temp gauge and one for the ecm. The one for the gauge generally has one wire going to it, the one for the ecm may have two wires going to it.

Are you sure its the gauge? A quick easy test is to ground the wire to the block. The gauge should sweep to HOT and peg. If it doesnt then the gauge cluster has to be replaced. Not real sure if you can purchase just the temp gauge in the cluster though. I think it comes as one complete package. This would require some dash removal. Not dificult but time consuming. If you want to go the cheaper route you can always mount a separate temp gauge.
You can check the temp. gauge operation by removing the temp. gauge sending unit plug and touching it to any engine ground with ignition on but the Van not running. The gauge will go to hot right away, if it is working. The temp. gauge sending unit is located on the drivers side of the engine at midpoint just below the exhaust manifold. It will have a single wire running to it and be plugged into the sending unit.
