To Remove either the front or rear brake rotor, you must first remove the tire and wheel to expose the caliper. Then you remove the caliper, there are two bolts, the heads facing outward toward you if you are at the wheel well. There is one bolt at the top, and one at the bottom. After removing the bolts, you can then take a large flathead screwdriver and pry the caliper away from the the rotor. The rotor will now slip of of the wheel lugs.
The minimum rear brake rotor thickness on a 1996 Lexus LS400 is 15mm.

Remove the wheel, remove the brake caliper, remove the brake rotor, disconnect wheel speed sensor from harness. remove the bearing hub, remove the wheel speed sensor from the bearing hub. Reverse order for install.
Remove the wheel, remove the caliper, and the rotor will just come off.

Break wheel nuts(tool: Wheel tool), jack up vehicle, Remove wheel, remove brake pads( tool: by hand or flat bit screw driver), then remove brake caliper( Tool: either 12,14,or 17 spanner or ratchet), after that you can remove the rotor by hand.

You would need to remove the brake caliper, brake caliper bracket and also the brake rotor. The cotter pin and nut that holds the axle in place also needs to be removed, and then the axle needs to be tapped out of the hub with a socket.
