There were two inquiries into the sinking of the Titanic.
The enquiry by the United States Senate was led by Senators William Alden Smith of Michigan and Isidor Rayner of Maryland. The findings are contained in report 806 of the 2nd session of the 62nd Congress.

They saw people around them screaming, crying, and even dying. They did not know if they were going to die or if their loved one was going to die. There are a few site that have quotes from some of the survivors (see Related Links).

Captain Edward John Smith was mildly scapegoated at the British Inquiry after the sinking of Titanic but it was only a matter of convenience to shift some blame to "the dead guy".
Many stokers made it to lifeboats, even lead stoker Frederick Barrett, who later testified at the American Inquiry.
Britain was very critical of the U.S. inquiry into the sinking of the Titanic. The ship was built in Britain and the British people felt as though the investigation should have been conducted by British people.
