For AC Soldiers, this is initiated at the MTF once a condition is brought to the attention of the hospital staff. For Reserve Soldiers, this is completed and signed by the doctor if the Soldier's medical condition resulted from military related activity. The Soldier then provides the completed form to his unit administrator to continue the process-hopefully ending in the govt both picking up the tab for the ER visit and added to the Soldier's medical record.
