While QID literally does mean four times a day, in practice qid meds are given at 0800, 1300, 1700, and 2200 hours (8am,1,5,10pm), in many places. This allows the patient time to sleep. D.D. -RN -----------------------------------

The "D" in "QID" means "daily".

Therefore, the subscription says to "ingest the medication by applying the medication to both eyes 4 times a day."
If this is some sort of a droplet, then you would usually see some sort of roman numeral added to the directions. For example iii ou qid... which would mean "3 droplets in each eye 4 times a day"...
Also, when it says "four times a day", we normally assume the medication is talking about a 24 hour interval... dividing 24 by 4 gives us 6; therefore, the medication should be applied every 6 hours.
Another way of writing "every 6 hours" would be "Q6H" or "q6h". You should confirm everything I just said with your pharmacist or physician.
