This is kind of opinionated. Early symptoms of Smallpox are Vomiting, Aches, High Fever, and Tiredness, while later symptoms are rashes and white legions. If I had to pick, I would say the white legions on the body are the worst symptom, as they can cover 80 percent of the body.
