If the lights are not working after using the remote start on a 2003 F-250, then something is wrong with the way it was wired when the remote start was installed. The remote start has wires to hook up to the lights so that a person knows the vehicle started. Perhaps this step was skipped in installation.
Google EHOW and they will give you exact directions on how to do it. It's kinda convoluted but does work.
remote starter will work then next time will not work

No - the starting system is totally separated from the remote entry.
This will 100% depend on the brand of remote start, it will come with instructions. On the other hand I like my system best, I take my car to the local stereo shop and have one put in. I would rather work on my race car than spend a frustrating day putting one of these in.
It will need to be re-programmed but, yes, it should.

(1) Have the batteries in them ever been replaced? (2) Check the fuse panel for a blown fuse controlling the circuit.
