Tousen began fighting Komamura and Hisagi, where Tousen revealed his new ability via Hollowfication.
Tousen then released his hollowfication against Komamura's bankai, giving him the ability to see. Komamura is quickly beaten down, but before Tousen can give the finishing strike, Hisagi stabs him in the head from behind.
Tousen's hollowfication release vanishes, reverting him back into his 'Vizard' like state. Tousen isn't as angry as he was, and just about manages to see Komamura and Hisagi for the last time, before his head explodes.

It looks like Ulquiorra kills Ichigo in an episode within 150-160, but he was only stabbed badly, and Orihime is able to heal me.
When Ichigo's Hollow self takes control, it means he was near death, and the hollow self has instant regeneration. This happens in his first actual fight with Byakuya (after he uses bankai and releases the true form of his zanpakuto), his fight with the bount (cant remember name), His fight with Zangetsu, during the season all zanpakuto betray their masters.
The only time Ichigo truly dies is by Ulquiorra in episode 269-270, when Ulquiorra greatly overpowers Ichigo after releasing, and showing that he is able to resist his Getsuga Tensu (Heaven Shocking, Lunar Fang). However, Ulquiorra decides to show no murcy, going into his form that makes him more powerful then Stark, and equaling Cero Yammy, claiming he is the only arrancar with a second release. It is proven he is dead (well, we know he is my goodness) when Orihime goes to Ichigo, although stopped by Ulquiorra, however he is encountered by Uryu, who knows the difference in power, but uses his speed and attack speed to keep Ulquiorra from attacking him, which works. During the fight, Orihime heals Ichigo, however has no effect. During Uyru's battle, he keeps a massive distance from Ulquiorra, firing massive amounts of arrows from any random direction, some being accurate, some being distracting arrows. However Ulquiorra is capable if blocking them with his wings easily. Uyru notices this, and tries to close in on him, giving him the upper hand until Ulquiorra gets a good chance to charge him, and knocks Uyru down into a pillar. Uryu sees how close Ulquiorra is and rushes himself to use his stronger arrows before he makes another move, but Ulquiorra goes *in unimaginable speeds* at Uyru, grabbing his wrist, and breaking it, then throwing Uyru outside. Then Orhime cries for Ichigo to save them, which brings him back to life in his hollow form. After his new form defeats Ulquiorra, he claims Uyru is assaulting him, and stabs him with his sword. As Ichigo goes to finish him off by charging a cero, Ulquiorra disrupts it and backfires on his body, which is what kills him.
