Ok I have asked this question in many different ways, and always seem to get the same answer. Blue Cross does cover the surgery however it is determined by your employer group if your plan cover the surgery. I was told the 85 percent of employers do not have the coverage in plan, since it will typiclly cause a 10 to 20 percent increase in their premiums.
