Definition

Diastasis recti is a separation between the left and right side of the rectus abdominis muscle, which covers the front surface of the belly area.

Causes, incidence, and risk factors

Diastasis recti is a common and normal condition in newborns. It is seen most frequently in premature and African American infants.

In pregnant women, increased tension on the abdominal wall may lead to diastasis recti. Multiple births or repeated pregnancies increase the risk.

Women who are 12 or more weeks pregnant should avoid aggressive abdominal exercises, which may worsen the condition.

Symptoms

A diastasis recti looks like a ridge, which runs down the middle of the belly area. It stretches from the bottom of the breastbone to the belly button, and increases with muscle straining.

In infants, the condition is most easily seen when the baby tries to sit up. It may not be seen when the child lies on the back and is relaxed. When the infant is relaxed, you can often feel the edges of the rectus muscles.

Diastasis recti is commonly seen in women who have multiple pregnancies, because the muscles have been stretched many times. Extra skin and soft tissue in the front of the abdominal wall may be the only signs of this condition in early pregnancy. In the later part of pregnancy, the top of the pregnant uterus is often seen bulging out of the abdominal wall. An outline of parts of the unborn baby may be seen in some severe cases.

Signs and tests

The doctor can diagnose this condition by performing a physical exam.

Treatment

No treatment is needed for pregnant women with this condition.

In infants, the rectus abdominis muscles continue to grow and the diastasis recti gradually disappears. Surgery may be needed if the baby develops a hernia that becomes trapped in the space between the muscles.

Expectations (prognosis)

The patient usually does very well. In most cases, diastasis recti usually heals on its own.

Pregnancy-related diastasis recti usually persists long after the woman gives birth. Exercise may help improve the condition. Umbilical hernia may occur in some cases. If pain is present, surgery may be needed.

Complications

In general, complications only result when a hernia develops.

Call your health care provider immediately if a child with diastasis recti develops redness or pain in the abdomen, has persistent vomiting, or cries constantly.

YES! To close abdominal separation you need to first build A LOT of strength in your deepest abdominal muscle, your Transverse Abdominis. Then you need to do specialized postpartum exercises that train the muscle to function properly as a stabilizer.

Women with diastasis recti should NOT perform ANY abdominal exercises that lift/roll the upper body off the floor or against the force of gravity as these types of movements make diastasis worse. This means no crunches, sit-ups, oblique pulses, roll downs/roll ups, Pilates mat work that uitlize the head-float position, and certain yoga asanas such as "boat pose."

  1. Aetna considers panniculectomy/apronectomy medically necessary according to the following criteria:

    1. Panniculus hangs below the level of the pubis; and
    2. The medical records document that the panniculus causes chronic intertrigo (dermatitis occurring on opposed surfaces of the skin, skin irritation, infection or chafing) that consistently recurs over 3 months while receiving appropriate medical therapy (e.g., oral or topical prescription medication), or remains refractory to appropriate medical therapy over a period of 3 months.

    Aetna considers panniculectomy/apronectomy cosmetic when these criteria are not met.

    Aetna considers panniculectomy/apronectomy experimental and investigational for minimizing the risk of hernia formation or recurrence. There is inadequate evidence that pannus contributes to hernia formation. The primary cause of hernia formation is an abdominal wall defect or weakness, not a pulling effect from a large or redundant pannus.

  2. Aetna considers repair of a true incisional or ventral hernia medically necessary.

In order to distinguish a ventral hernia repair from a purely cosmetic abdominoplasty, Aetna requires documentation of the size of the hernia, whether the ventral hernia is reducible, whether the hernia is accompanied by pain or other symptoms, the extent of diastasis (separation) of rectus abdominus muscles, whether there is a defect (as opposed to mere thinning) of the abdominal fascia, and office notes indicating the presence and size of the fascial defect.

Diastole is the phase in the cardiac cycle that allows filling of blood into the ventricle. It corresponds to the lower number in the blood pressure measurement. A normal upper limit of the blood pressure falls in the range less than 120/80 mm Hg. Above this value, it is considered pre-hypertension based on The Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure.

During the diastolic period the ventricle is in the fifth phase of the cardiac cycle. This phase is termed isovolumetric relaxation which occurs immediately after the second heart sound. In this phase, all valves of the heart are closed. The volume of blood in the ventricle during this phase of the cardiac cycle remains constant. Following the fifth phase of the cardiac cycle is the rapid filling of blood into the ventricle. This cycle corresponds to the opening of the mitral and tricuspid valves located between the left and right atria and ventricle, respectively. The next phase of the cardiac cycle, called diastasis, is the longest phase of the cardiac cycle. In this cycle, the ventricle continues to fill with blood but at a much slower rate. The final phase of diastole corresponds to the first phase of the cardiac cycle. In this phase of the cardiac cycle, the atria contracts to provide additional filling of blood into the ventricle. After the final phase of diastole, the systolic phase of the cardiac cycle begins.

Definition

Beckwith-Wiedemann syndrome is a congenital (present from birth) growth disorder that causes large body size, large organs, and other symptoms.

Causes, incidence, and risk factors

The cause of Beckwith-Wiedemann syndrome is unknown, but it may be genetic. Most cases are associated with a defect in chromosome number 11.

Infancy can be a critical period because of low blood sugar (hypoglycemia), omphalocele (when present), and an increased rate of tumor development. Wilm's tumor and adrenal carcinoma are the most common tumor's in patients with this syndrome.

SymptomsSigns and tests

The signs of Beckwith-Wiedemann syndrome include:

  • A ridge in the forehead caused by premature closure of the bones (metopic ridge)
  • Enlarged fontanelle (soft spot)
  • Enlarged kidneys, liver, and spleen
  • Large size (90th percentile)
  • Low blood sugar (hypoglycemia)

Tests for Beckwith-Wiedemann syndrome include:

Treatment

Infants with low blood sugar may be treated fluids given through a vein ( intravenous solutions).

Defects in the abdominal wall may need to be repaired. The child must be watched closely for the development of tumors.

Expectations (prognosis)

Children with Beckwith-Wiedemann syndrome who survive infancy do well, although no long-term follow-up information is available. Mental development appears to be normal to very slightly decreased. Swelling of the tongue can cause problems with feeding and sleeping.
