AstraZeneca claims that esomeprazole provides improved efficacy, in terms of stomach acid control, over racemic omeprazole. Many health professionals have expressed the view that this improvement in efficacy is due to the dose of esomeprazole recommended for therapy rather than any superiority of esomeprazole per se.
An alternative rationale suggested for the use of Nexium was the reduction in interindividual variability in efficacy. However the clinical advantage of this hypothesis has not thoroughly been tested in large-scale trials.
The weirdness of the efficiancy of Nexium is that some factors showed higher efficiancy in humans while other factors showed higher in only rats and dogs. Clinical trials cannot concretely prove the efficiancy; purely depends on the person.

Prilosec is "Omeprazole" or a proton pump inhibitor used for the treatment of dyspepsia, peptic ulcer disease (PUD), gastroesophageal reflux disease (GORD/GERD) and Zollinger-Ellison syndrome. It was first marketed in the US in 1989 by AstraZeneca as the magnesium salt omeprazole magnesium under the brand names Losec and Prilosec, and is now also available from generic manufacturers under various brand names. Omeprazole is one of the most widely prescribed drugs internationally and is available over the counter in some countries.

An at-risk launch, in the context of pharmaceuticals, is when a generic manufacturer challenges the validity of an existing patent.
If the company were to launch the drug before this first round of litigation and lose (ie. the patent is upheld), they would owe the patent-holder treble damages. This would be roughly three times the sales loss incurred to the patent-holder, and no small change to even a large company.
If, however, the court rules that the patent is invalid then the newcomer might choose to launch the product "at risk": while the erstwhile patent-holder appeals the initial court decision not to uphold the patent.
Ultimately the court will stand by its decision and the newcomer will have successfully launched at-risk, or it will revert back and support the appeal. In this situation, the newcomer only owes 1x damages. This is a considerably less prohibitive sum, one that theoretically begot the original risk-reward question of launching the product despite the appeal.

Many drugs are not invented - they already existed before man showed up, so they are really "discovered". For example, the resin from the opium poppy, cannabis, tobacco, coffee (caffeine) and a whole raft of other plant based drugs that include everything from things like belladonna (deadly nightshade) which is used for heart treatment, the bark of the willow tree (same active ingredient as aspirin) through to khat - and coca leaves (cocaine albeit in a crude state).
So, some drugs are discovered, some are made artifically - but are you after the name of the person who made the first synthetic drug or an exhaustive list of people who invent or make them?

Technically, there is no generic drug for Nexium (generic name is esomeprazole). However, esomeprazole is the isolated isomer (S-enantiomer) of omeprazole which you may know more as the over-the-counter drug Prilosec.
Omeprazole and esomeprazole are both proton pump inhibitors, used to treat duodenal and gastric ulcers and gastro-esophageal reflux disease (GERD).
When the patent was running out for omeprazole, AstraZeneca, the manufacturing company of omeprazole, developed the isolated form as a way to counter the loss of profit it would incur after the expiration of its patent. This practice is common among drug companies foreseeing the expiration of its major drugs.
AstraZeneca marketed Nexium ("the purple pill") as a more potent and overall more effective drug than omeprazole. In actuality, although the S-enantiomer is more active than the R-enantiomer, the difference does not account for much. In general, the difference between a single active enantiomer and a mixture can be a difference in time of onset of action (sometimes 5 minutes) or it can be the strength of the dose (double the strength of the mixture). Compared to the relatively low cost of omeprazole, the more expensive cost of Nexium is not worth the differences between the two drugs. It may still be disputable however. Some people believe the active enantiomer does in fact provide better effectiveness than the racemic mixture.
