The Port Authority Bus Terminal (42nd Street at 8th Avenue) is the terminus for Greyhound buses. You can walk 1 block east on 42nd Street to 7th Avenue, and enter the subway at the 42nd Street-Times Square station, or you can enter the subway system at 42nd Street-Port Authority, and then walk through the tunnel that connects the 42nd-Port Authority station to the 42nd-Times Square station.
Take the Uptown 7 train (the purple line), 1 stop, from 42nd Street-Times Square to 42nd Street-Bryant Park. Transfer at Bryant Park to the Uptown F train (of the B-D-F-M, the orange line). Take the Uptown F to the end of the line, the Jamaica-179th Street station.

Well, you could take the subway, but it's not a long walk. Only about a mile. 125 West 45th Street is between 6th and 7th Avenues. So, you just walk west on 42nd Street (Grand Central is on the north side of 42nd, and traffic on the north side of the street flows west, so you just follow the direction that the traffic is going on the side of the street you are on). Walk west on 42nd until you get to 6th Avenue. Turn right onto 6th Avenue, and walk north to 45th Street. Then turn left onto 45th between 6th and 7th.
If you want to take the subway, then take the S shuttle train (color code grey) east across 42nd Street, from Grand Central (East 42nd) to Times Square (West 42nd).
You will exit the subway at West 42nd and 7th Avenue. Walk north on 7th to 45th, and then make a right onto 45th between 7th and 6th.

Grand Central Station is on Lexington Avenue and East 42nd Street. Penn Station is between 7th and 8th Avenues, from West 33rd to West 34th Street.
When you exit Grand Central Station, walk west on 42nd Street. Forty-second Street is a two-laned street that runs west to east and east to west. The north side of 42nd Street is the side that goes east to west, and Grand Central Station is on the north side of 42nd Street. So, to go west, you follow the direction that traffic is going on the side of the street you are on.
Walk west until you get to 7th Avenue and West 42nd Street. At 7th Avenue, turn left, and walk south to 33rd Street. The east entrance to Penn Station is on 7th Avenue and West 33rd Street.
The total walking distance is about a mile and a half.
